I wonder why:
1) The pots are linear?
2) The pots and resistors are rated for 2 Watts? Perhaps in case some well-meaning dufus tries to drive speakers with this?

Sorry for reviving this ancient thread, but I've been considering a DIY solution like this for a long time and wondering what value pots I should be looking for to put between the power amp and the headphones?

Not sure what a 1:1 transformer will achieve on a mic output, but hey why not?

The CMOY is barely a "design." Just run the inputs to a dual opamp and the outputs to a pair of headphones. Since you've got a bipolar supply handy you can even skip the virtual ground part of the circuit. With +4dB inputs you'll probably want a bigger value resistor in the feedback path for less ga...

When going from unbalanced to balanced, you want to connect hot and ground from the TS jack to 2 and 3 of the XLR. Connect XLR pin 1 to the shield on the XLR end, but leave it unconnected at the unbalanced end.

Hey Tom, long time no chat!

Seems like there's any number of high-quality differential opamps that would be happy with a line-output source impedance without the need for input transformers. Have you considered making the inputs passive? Or are you looking to have pan control as well.

A reamp box is even easier if you leave out the volume and impedance controls, both of which I find to be unnecessary. Then you're basically left with this:
