Unplugging and plugging back in on a monitor that is on and running does not sound at all pretty. You can pop a tweeter that way too.
Turning the speakers off before swapping cables happened every time. Only crazy people would do otherwise.

Rule number One for basic troubleshooting - Eliminate Variables. I would start by using the same mic and cable plugged directly into the mixer and the headphones plugged directly into whatever output is driving the headphone amp. Can just borrow a drummer?

If not, try a different drummer. It's a matter of micro-seconds if a drummer and bassist can lock a groove. If they can, you can do no wrong.

Cleaning and working the send/returns is good. Try putting a short patch cable in the inserts, from send to return. Sometimes the switch in the return jack gets corroded and won't provide clean signal.

Nothing wrong with recording live to stereo. And, the equipment used to play is more or less irrelevant if the spirit is there. I neglected to mention the jacks but clean all of them, including the plugin for the power supply if there is one.
Long overdue anyway, no harm done if it doesn't fix anything but it may just sound better with clean jacks.

I would want the bass and the drums to play together on the initial recording. That's how groove is made, the visual connection is huge. Most listeners tune in the vocal/story and the groove and the rest of it is fine as long as it doesn't interfere. Pretty different from how we tend to think of rec...
