Sampling is such a broad and undefined term as to be almost useless. However, comparing it to doing cover tunes is completely missing the point and shows a lack of musical sophistication and understanding.

We don't just need new copyright laws, we need a new vocabulary.

I would think "produced by" covers it all, in the absence of an engineering credit going to someone else. If you produced it, and there's no other recording/mixing etc credits, the assumption, in my mind anyway, would be you did it all. Not necessarily - many older recordings only credit the produc...

Where they go depends a lot on the logistics and the situation.

In some situations there is enough audience on the stage mic's, but it is always nice to have dedicated mic's for applause and crowd response when possible.

Cards, Omni's, even shotguns - it depends on the situation.
