Are you using old tape? I've had this with old 456 from the 90s, lots of speed stability problems. Some fresh tape and a good cleaning took care of it.

Want to really mess your head up? Stick a nail somewhere near the top of the akai, between the two reels to guide some tape over and splice together a loop that fitsaround the heads and over the nail. Record some stuff for long enough to cover the entire loop of tape. You might need to use some duct...
