45 ft should be no problem for a non amplified dsub run, i regularly run 50-100ft vga cables on everything from a netbook to a pretty burly desktop i use for projection design stuff without worrying about signal loss at all. so my guess is that it's the cable, maybe a bad pin configuration or someth...
