On the 'scope test, I had nothing connected to the input. But the noise was evident during just listening with a mic or line signal plugged in--that's where I first noticed it.

Should I ground the inputs to do the 'scoping, or?

I would check the power input pins 7 and 4 with a scope. If you see ripple (lines not wine) the power supply is getting pulled to hard or lacks sufficiant regulation/filtering.
