Very simple repair. Might not fix your issue, but it's probably worth a try.

I bought a factory refurbished R16 about two months ago and it's been rock-solid for me.

Steel wool on chrome will scratch it. Use one of those 3M green synthetic scrubber pads instead. BarKeeper's Friend works well for soaking rusted/dirty metal. FWIW, the pm1k input transformers are not 600:600 ohm. They're more like 150:10k (or something like that) mic input transformers, and will probably saturate with a line level input.
