Maybe somebody installed an potentiometer with an inappropriate taper. Pots for audio are normally a logarithmic taper. I can't say that I have heard the effects of linear taper pot within an audio circuit, but perhaps that is what you are experiencing. Sorry for answering without a definitive answer.

Based on what I have read in the Pro Tools 8 Reference manual, I would guess that the mix buss is not dithered until you apply dither (via a plug-in) to the Master Fader track.
