Me, I include the player, instrument and gear recording chain in the name of the individual track - look to the lyric drop downs on most all songs on my bandcamp and you can find 'em listed.
My lyric sheets - whether my lyrics or the artists I'm recording, I keep 'em digitally, usually as .txt - list the chains plus important settings, ex., "compressor 12@1 at 12dB". That kind of notation works for me because I am in the habit of bringing all my tracks up to peaking at -3dB, or RMS -6dB.
Very rarely do I use screenshots or photos - the latter more just to show off the gear vs. for any particular settings, altho' I do use 'em for mic placement.
Oh, and I sometimes list the intoxicants used, if any.

I have spiral notebooks full of notes, but its all from my own recordings.
